Individualized Education Plan and Transition

Courses of study, present levels of educational performance, transition services and transfer of rights are entered into the individualized education plan. Student input is vital. The Postsecondary Transition Plan (PTP) addresses the following factors in the IEP related to transitioning to community activity and integration after high school:
