Transition

Transition is the process of preparing to change from one environment to another. It should be a partnership between the student, family, school and post school service personnel. Some of the agency resources available are: Department of Vocational Rehabilitation, County Human Services, College/Technical school representatives, Independent Living Center staff and Job Center staff. It can include others that will be necessary to assist the student in assuring that all needs are addressed.
